What is changing?
Critical Illness with Cancer (Unum) offers more robust coverage with benefit options in $10,000, $20,000 and $30,000 increments. (Must be deemed cured of cancer if previously diagnosed). With the addition of cancer coverage to Critical Illness, we are ending current coverage with Allstate effective 12/31/2018. Employees that choose to keep their current Allstate Policy will have the convenience of payroll deductions for premium payment.
Accident Plan Design (Unum) offers reduced premiums with higher reimbursement rates for ER visit. Accident Insurance can pay a set benefit amount based on the type of injury you have and the type of treatment you need. It covers accidents that occur on and off the job.
ARAG is our new carrier for legal services for Power of Attorney, Wills, Real Estate matters, Traffic Infractions and more. Two plans are offered: Base Plan (no divorce) and Enhanced Plan with Divorce and Identity Theft Coverage. Learn more and see schedule of covered services >
Supplemental Life Insurance (The Hartford): Employees currently enrolled in the Supplemental life plan can increase 2 increments ($20,000) without EOI (Evidence of Insurability) as long as the new amount does not exceed the GI (Guaranteed Issue of $150,000) or 5 times your base salary. Employees that currently do not have Supplemental life coverage can elect 2 increments ($20,000) without EOI. If an employee was previously declined they are not eligible without EOI. For spouse coverage, those currently participating can increase 2 increments ($10,000) without EOI as long as the new amount doesn’t exceed the GI of $30,000 or 50% of employee coverage. Those not participating can elect 2 increments ($10,000) without EOI. If a spouse was declined in the past they are not eligible without EOI.
Child Life Benefit (The Hartford) increased from $5,000 to $10,000 for a premium rate of $0.23 each pay period.
Urgent Care Co-pay of $75 remained the same for AppUrgent Care. Co-pays for all other urgent care facilities increased to $125.
403(b) & Roth Combined Limits up to the 2019 established IRS amounts, which have not been released. Once released, the limits will be shown on the Retirement benefits page

Form 5500 Filings with the Department of Labor
Each year ARHS is required to submit the Form 5500 on employee benefit plan’s annual information with the Department of Labor. The filing has been completed for the Plan year 2016. As part of the annual reporting requirements, ERISA, (Employee Retirement income Security Act of 1974), requires that ARHS provide plan participants with a copy of the Summary Annual Reports for our benefit plans.
